Achenbach Child Behavior Checklist (CBCL) General Information

There are two ways to complete the Achenbach Child Behavior Checklist (CBCL). Online or hardcopy with pencil and paper. You can use a paper form entitled Child Behavior Checklist for Ages 6-18 for children age 6 and above, but it is easier to complete these online. For children age 4 & 5, you must use a paper form entitled Child Behavior Checklist for Ages 1 ½-5. This form is also used with children who are less than 48 months of age and are medically fragile or have special needs requiring a level of care. WHO: Persons who complete CBCLs should be adults who know the children well, such as parents, foster parents, or State Social Workers; but never the children!

WHEN: Initial LOCs or reassignments require CBCLs be submitted with other information, if the IQ is 70 or above. Once children are in DCBS foster care or PCC residential or foster care placements, CBCLs are to be completed every 6 months with the "Application for LOC Payment." For children with an IQ below 70, the Reiss is to be submitted with the Application for Level Payment as required.

REPORTING PERIOD: On the form Child Behavior Checklist for Ages 1 ½-5 report behavior happening now or that happened within the past 2 months. Do not report behavior if it happened before 2 months ago. On form Child Behavior Checklist for Ages 6-18 report behavior happened now or that happened within the past 6 months. Do not report behavior that happened more than 6 months ago, no matter how dramatic or important that behavior was.

WHY: As required by 922 KAR 1:360, Levels of Care are determined from two sources, the narrative DPP-886A or "Application for Level of Care Payment" and an objective behavior inventory. The latter is the Achenbach Child Behavior Checklist (CBCL) for children with an IQ of 70 or above.
