Monthly Census and Foster Care Child Link Reports
Purpose
To provide the daily location of CHFS youth in out-of-home care, information about
approved PCC foster parents, discharge data which are utilized to create outcomes and
comparative reports, and information regarding important action dates for PCC and DCBS staff.
Recipients
The Monthly Census Reports is sent to the Residential Treatment Programs and DCBS workers.
The PCC Foster Care Child Link Report (CLR) is sent to PCC Foster Care Programs.
Provider Responsibilities
- Residential Treatment
- If a child has been discharged from your program, record the information in
the space provided on the MCR, including date discharged,
reason for discharge,
and the complete name of the new placement (including county, if foster care).
- If a child has moved and come back to your program within the month, provide
the dates the child was not in your program and the specific name of the placement
where the child was located (e.g., ABC TFC Fayette county, Warren County Detention
Center, AWOL))
- If a child in your care is not listed on the MCR, but you know the child is committed
to CHFS, provide the child's name, social security number and date of admission on the MCR.
- If the child's DCBS worker has changed, please write this on the MCR. Include the
worker's full name and their county office.
- If there are no corrections, mark "OK" on the MCR and return to CRP.

- PCC Foster Care
- If a child has been discharged from your program or changed foster homes within
your program, record the discharge information in the space provided on the CLR,
including date discharged, reason for discharge,
and the complete name of the new placement, and county (if foster care). For newly
admitted CHFS committed youth or youth transferred within your foster home system,
complete the Child Admission-Discharge Form.
For the CLR, you will also need to update information on the PCC foster parents on the CLR.
- If a child has moved and come back to your program within the month, provide the dates
the child was not in your program and the specific name of the placement where the child
was located (e.g., ABC TFC Fayette county, Warren County Detention Center, AWOL).
- If you need to add new foster homes approved by your agency complete the
Foster Parent Information Form.
- If you have been licensed as a Medically Fragile Home and have a foster home
certified as a medically fragile home, you will need to complete the
Private Child Care Foster Care Medically Fragile Checklist.
- If the child’s DCBS worker has changed, please write this on the CLR. Include the worker’s full name and their county office.
- If there are no corrections to the youth admitted to the PCC foster home or the foster parent information, you do not have to submit the CLR for that foster home. Only submit those CLR report pages that need updated.
