Connections (Lexington and Somerset)
provides services for individuals with intellectual or
developmental disabilities through the Supports for Community
Living (SCL) and Michelle P. Waiver programs. These services
are designed to support individuals in becoming more independent
by encouraging personal growth and development and increasing
involvement in the community.

Access is the central point of intake for
non-clinical services for persons with intellectual disabilities
and/or developmental disabilities. Access case managers promote
full participation in community life by coordinating and
developing supports within the community for individuals with
intellectual disabilities and/or developmental disabilities.

of homelessness, institutionalization and/or

First Steps is Kentucky’s Early
Intervention System that serves children from birth to the age
of three who have developmental delays or a medical condition
that is known to cause a delay. The Bluegrass First Steps Point
of Entry office provides eligibility determination for all
referrals, as well as service coordination for families who have
children in the First Steps Program.
Service Coordinators will:
- Guide families through the First Steps
program
- Coordinate developmental evaluations
and determinations regarding eligibility
- Coordinate the initial discipline
specific therapy assessments
- Assist family with obtaining on-going
therapy for their child, if eligible
Therapists provide therapy in the child’s
natural environment (home, daycare and community). This allows
parents or caregivers the opportunity to learn activities that
promote the child’s development.
Therapy services may include:
- Speech Therapy
- Physical Therapy
- Occupational Therapy
- Developmental Intervention
- Behavioral Intervention
- Nutrition Services
- Services for Deaf and Hard of Hearing
- Services for Blind and Sight Impaired

age of three is not meeting his/her developmental milestone, you
